Wild Child in the PACU: Update on Emergence Agitation

چکیده

Introduction Though postoperative excitement has been a feature of general anesthesia since before the development of most of our modern inhalational anesthetics, the number of studies examining the incidence of postoperative agitation has increased exponentially since the introduction of the poorly soluble inhalation agents sevoflurane and desflurane into routine anesthesia practice. Eckenhoff first described the phenomenon of emergence delirium in 1961. (1) In a review of 14, 436 postoperative patients he found four factors to be associated with a statistically higher incidence of emergence agitation: 1) children 3 – 9 years (13%), 2) cyclopropane or ether anesthesia, 3) tonsillectomy, and 4) barbiturate premedication. He also found that the incidence of emergence reactions was reduced in patients who had received intraoperative opioids. Though cyclopropane and ether anesthetics are no longer used, recent studies have confirmed the accuracy of many of Eckenhoff’s earlier observations as causative factors for emergence phenomena.

A comparative study of emergence agitation between sevoflurane and propofol anesthesia in adults after closed reduction of nasal bone fracture

BACKGROUND Emergence agitation is associated with increased morbidity and hospital costs. However, there have been few reports in the medical literature on the occurrence of emergence agitation in adults. The aim of this study was to compare emergence agitation between sevoflurane and propofol anesthesia in adults after closed reduction of nasal bone fracture. METHODS Forty adults (ASA I-II, ...

The effects of midazolam administered postoperatively on emergence agitation in pediatric strabismus surgery

BACKGROUND The present study tested the effect of midazolam administration after sevoflurane anesthesia against emergence agitation in children in the recovery phase. METHODS A total of 60 children presenting for ophthalmic surgery under sevoflurane anesthesia were randomly placed in four groups from Group I to Group IV. Before the end of the surgery, we injected normal saline 2 ml in Group I...

Comparative study of emergence agitation between isoflurane and propofol anesthesia in adults after closed reduction of distal radius fracture.

Distal radius fracture is a common wound. It is reduced by surgery under anesthesia. Emergence agitation can often occur after anesthesia. It is associated with increased morbidity and hospital costs. However, there have been almost no reports in the medical literature on the occurrence of emergence agitation in adults. This study aimed to compare emergence agitation between isoflurane and prop...

Emergence agitation after sevoflurane anaesthesia in children.

Editor—We read with interest the article by Lee and colleagues comparing propofol and fentanyl for prevention of emergence agitation after sevoflurane anaesthesia in children. We thank the authors for their work in this very common and often very distressing scenario. We would like to raise a few important questions regarding this study. Pain is one of the important causes for emergence agitati...

The effect of ketamine on the incidence of emergence agitation in children undergoing tonsillectomy and adenoidectomy under sevoflurane general anesthesia

BACKGROUND The rapid emergence and recovery from general anesthesia afforded by sevoflurane is associated with a high incidence of emergence agitation in children. Small doses of ketamine reduce the incidence of emergence agitation. This study compared the effects of ketamine 0.25 mg/kg and 0.5 mg/kg on emergence agitation and postoperative pain. METHODS The effects of added intravenous ketam...
